I agree to accept tutoring/ teaching from Another Chance for Academic Excellence (ACAE) under the terms and conditions listed below:
Communication #1
I agree to:
*Contact ACAE at least 48 hours in advance to arrange tutoring sessions.
*Complete all forms and documents before tutoring begins.
*Accept tutoring on a first come, first serve basis.
*Arrange to meet at a mutually agreed upon time and location for tutoring (zoom, library, school, etc.)
Communication #2
I agree to:
*Prepare questions and identify material to preview prior to the scheduled tutoring session in-person, via email, or Zoom.
*Provide ACAE with at least 24 hours notice if a scheduled tutoring session must be canceled or services are no longer needed.
*ACAE terminating the tutoring contract immediately if two scheduled tutoring sessions are missed consecutively without prior notice and forfeit service for 1 month.
Basic Account Information
I agree to:
*Provide ACAE with parent/guardian's name(s).
*Provide ACAE with student(s) name(s), NO NICKNAMES.
*Provide ACAE with a working phone number.
*Provide ACAE with a working email address.
*Provide ACAE with a physical address.
Payment
I agree to:
*Submit 1/2 of payment before the tutoring session, and the balance of the payment the day of the session, or submit total payment before the session or activity. Other arrangements may be made. (A receipt will be produced either online or paper.)
Motivation
I agree to:
*Give my child encouragement and motivation to accomplish the academic challenges placed before him or her. Be your child's greatest cheerleader!
*NOT protect my child from new tests!
Cooperation
I agree to:
*Speak respectfully and calmly with the tutors and teachers for the betterment of the student's academic development.
